All persons must sign this release prior to being involved in any activity associated with Habitat for Horses.

  • If you are 18 years of age or older, you may sign a Release when you have read and understood it completely.
  • Minors, being those under 18 years of age, must complete this form and have it signed in advance by a parent or legal guardian.
  • All signatures must be witnessed.
  • Please make sure you read every section carefully and the entire release is completely filled in, otherwise you will not be able to participate in any activity or enter upon any property under the control of Habitat for Horses.
